“Venue” refers to 342D King George’s Avenue, Singapore 208575
“Keiseikan” refers to Keiseikan Martial Arts LLP located at the “Venue”.
“Activities” refers to physical, karate and kobudo training.
Indemnity
I hereby certify that I have voluntarily signed up myself or my child to participate in the “Activities” at “Keiseikan”. I hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my participation in the Activities, and do hereby release and forever discharge “Keiseikan’s” managers, members and volunteers for any physical or psychological injury, including but not limited to illness, paralysis, death, damages, economical or emotional loss, that I may suffer as a direct result of my participation in the aforementioned Activity, including traveling to and from an event related to this Activity.
I acknowledge that this “Activities” may involve a test if a person’s physical and mental limits and may carry with it the potential for minor or serious injuries or death. The risks may include but are not limited to, those caused by the condition of equipment and actions of others, including but not limited to coaches, participants and/or volunteers. In the event that I should require medical care or treatment, I agree to be financially responsible for any costs incurred as a result of such treatment. I understand that “Keiseikan’s” coaches will take all necessary safety measures during the Activities and am aware and understand that I should carry my own health insurance.
I agree to comply with all rules and regulations set forth by Keiseikan and to follow all instructions provided by the coaches, trainers, and other sports personnel. I acknowledge that failure to do so may result in injury or harm to myself or others.
I hereby release, waive, discharge, and covenant not to sue Keiseikan, coaches, trainers, and other sports personnel for any and all liability, claims, demands, actions, loss or damage of personal belongings, including death that may be sustained by me or any other person while participating in the activities.
I agree to indemnify and hold harmless Ketseikan, coaches, trainers, and other sports personnel from any and all claims, demands, and expenses arising out of or related to my participation in the sports activities, including any claims made by third parties.
I have read this disclaimer and fully understand its contents. I agree to be bound by its terms and acknowledge that it constitutes a complete and unconditional release of liability and waiver of legal rights.
